The Show Your Love Initiative has tested and developed a series of materials to help people learn more about wellness topics. These are printable, sharable resources for providers and health teams to distribute, and for consumers to use to learn more about their health, annuals visits, and resources to take action.

The Know Your Numbers booklet covers typical annual visit check-up tests, such as blood pressure, height, weight, heart rate/ health, urine samples, blood tests, and more. Each page breaks down what different tests are, why they’re important, healthy ranges to know about, and resources to consider. Each page has space for a patient to write down questions or concerns and for providers to write information for the patient.
